Before your appointment, please complete our intake forms in full, signing where necessary. Our intake forms are provided through PtEverywhere. You will receive emails from us describing how to access and complete these forms after scheduling your initial evaluation. You can access PtEverywhere through a web browser or by downloading an app for your mobile device.

What You Need to Know

Many of your general questions and billing information will be addressed on our main IPA Physio General FAQ page and General Billing & Rates page. The information below addresses unique regulations related to our Portland location.

We are an out-of-network (OON) provider. Insurance benefits vary, but you may be responsible for the entire cost of treatment. If you are unsure of your OON benefits please contact our office so that we can verify your benefits.

– The typical recommended initial course of treatment is between 4-6 treatment sessions.
– Every appointment is 1 hour one-on-one with a physical therapist, so that you receive the best possible care and expedite your recovery.

Oregon is one of 20+ states in the United States that have unrestricted direct access to physical therapy services without the need of a prescription from a referring provider. You are allowed to come directly to physical therapy without having to consult another medical provider. Some insurance companies may require a referral for reimbursement. We also enjoy working in conjunction with other medical providers, so we may ask you to get a consultation with another medical provider if we believe the referral is warranted or would improve the quality of care you are receiving. The Physical Therapy Practice Act in Oregon mandates that we discharge a patient after 30 days of not being seen at IPA Physio for physical therapy services. After a patient has been discharged, we are required by law to do a new evaluation.

There is an exception if you and your physical therapist:

  • Plan on following up in a certain amount of time as long as it is documented in your file
  • Schedule the follow up appointment at the time of your last visit

We are licensed physical therapy professionals by the Oregon Board of Physical Therapy. According to our professional license, we are required to perform an evaluation on any patient seeking physical therapy treatment or treatment modalities (i.e. cold laser therapy), prior to providing any patient care. We understand it may seem like an unnecessary formality to you, but it is a mandated requirement.

Yes. In some cases IPA Physio Portland will accept workers comp cases or liens. Please contact us to inquire if we can accept your case.

SOON! In Oregon it will be explicitly permitted for physical therapists to perform dry needling as of July 1, 2027, following the passage of House Bill 3824.

We do not use ART specifically at IPA Physio Portland. However, there are some similarities between Functional Manual Therapy® and ART. If you have been told that you would benefit from ART, or have benefited from ART in the past, FMT will help take you to the next level.
